EMPLOYEE BENEFIT GUIDE

These benefits are only provided to home health aides who are servicing cases that are reimbursed
by Medicaid.
Every employee has already received, or will receive, a card in the mail that looks like a credit card.
This card is an Employee Benefit Card and can be used as a credit card to purchase transit tickets
(MetroCard), pay your cellphone bill, pay deductibles and copayments, cover dental or vision services
expenses.
Each month, All Heart Homecare analyzes the hours worked for each employee and pre-deposits money
electronically onto the Employee Benefit Card. Please use your zip code when requested.

1. Transit and Cellphone Benefit

One of the most valued benefits is covering your travel and cell phone expenses. Every employee at All
Heart will earn funds based on the hours worked per month. For example:

  • Parking Expenses: Any expenses incurred to park your vehicle for a work purpose;
  • Transit Pass: As discussed above, you can use the transit credit card to purchase MTA MetroCard. You can also use the card to purchase tickets on the LIRR, Metro-North or any mass transit facility such as UberPOOL and Lyft Shared.
  • Cell Phone expenses: You can use your cell phone credit to pay for your cell phone expenses at the carrier of your choice.
2. Flexible Spending Account (FSA)

A Flexible Spending Account (also known as a flexible spending arrangement) is a special account that
you use to pay for certain out-of-pocket health care costs. You don’t pay taxes on this money. Every
employee at All Heart will earn funds based on the hours worked per month. For example:

  • You can use funds in your FSA to pay for certain medical and dental expenses for you, your spouse if
    you’re married, and your dependents.
  • You can spend FSA funds to pay deductibles and copayments, but not for insurance premiums.
  • You can spend FSA funds on prescription medications, as well as over-the-counter medicines with a doctor's prescription. Reimbursements for insulin are allowed without a prescription.
  • FSAs may also be used to cover costs of medical equipment like crutches, supplies like bandages, and diagnostic devices like blood sugar test kits.
3. Limited Scope HRA (Health Reimbursement Account):

The limited scope HRA is an account which enables employees to make purchase or cover costs
associated exclusively with Dental or Vision services and products. New glasses, teeth cleanings, co-
pay or deductible associated with getting a cavity filled, costs for eye-exams, are all examples of costs
that can be paid through the limited scope HRA benefit account.

  • This plan can be used to reimburse out of pocket dental and vision expenses only (no medical);
  • Does not have to be connected to a group or high deductible medical plan;
  • These are employer contributions for limited use;
  • Does not cover any procedures that are cosmetic in nature.

Managing Your Account Online

Clarity Benefit Solutions offers an up-to-the-minute website to help you manage your account.  You are able to review your balances and transactions, as well as, submit manual claims for Cell Phone reimbursement. You must use your Clarity Convenience Card for Mass Transit expenses. Please read the following for tips on creating an account online:

Instead of our benefits, you can choose HIP EmblemHealth insurance. Monthly payment for an individual plan is $149.20.

In order to apply:

  1. It is necessary to work at least 33 hours per week (130 hours per month) to enroll.
  2. You can sign up:
    • In 90 days after hire date;
    • If you have lost your insurance;
    • During open enrollment period (November).
  3. It is necessary to notify the agency at least 2 weeks in advance if you would like to enroll.

All Heart Homecare Agency would like to introduce you to our new training and employee development platform called showd.me! This platform provides straight-forward instruction and up-to-date in-service training. Thus, you will now be able to complete the in-service training in the convenience of your home or on-the-go, at your own pace!

We remind you that you are required to pass 12 hours of in-service each year (3 hours/4 times a year). If you skip the in-service, you will not be able to work.

The In-service will take a total of 3 hours to complete. You are not required to complete the In-service in one sitting. Your work will automatically save once you sign out and will pick up where you last left off when you sign back in.

A 401K is a retirement savings plan sponsored by an employer. It lets workers save and invest a piece of their paycheck before taxes are taken out. Taxes aren't paid until the money is withdrawn from the account.

You can be eligible for 401K plan once you have been employed with the company for one full year.

What is the Agency’s pay schedule?

Weekly pay cycle consists of 7 working days, beginning on Saturday and ending on Friday of the pay period. You will be paid every week, on Friday.

When can I take a vacation/sick time? Will I be paid for it?

Vacation/sick time is being accumulated based on the hours worked. Full time employees are eligible for 40 hours of paid time off per year. There is a 120-day waiting period from the date of hire before you can start redeeming accumulated hours. Your time off will be paid according to the pay schedule.

If you are planning a vacation, you should notify the coordinator at least 2 weeks in advance (in the summertime - at least one month in advance).

How to set up Direct Deposit?

To set up Direct Deposit you need to send us a Direct Deposit Set-Up Form from your bank or fill out our form and attach a copy of a voided check.

How can I view my pay stubs and W-2?

To view your pay stubs online you need to sign up for Viventium program.

To sign up:

  1. Provide us with your e-mail address;
  2. Receive the confirmation e-mail from us and reply to it;
  3. Afterwards, you will receive a Viventium registration link;
  4. Register on the Viventium online portal.

How to update my address?

To update your address, you need to fill out a Change of Address form in our office.

Employment letter

Employment letters are prepared every Friday. Please contact the Agency in advance to request a letter.

 

Why do I need the flu vaccine every year?

The flu virus mutates and a new vaccine is developed every year. Thus, you need to receive a flu shot every Flu Season.
