• Work Hours. The employee will report to work promptly at the time assigned and will not change the assigned hours withoutthe prior consent of the mc office. Failure to report to work on time, to properly notify the AGENCY office of intent not to work an assigned shift, or refusal or failure to work the designated hours is a violation of the AGENCY policy. Employees are not permitted to leave their client’s home early without the prior consent of the AGENCY office, and if this is done it will be determined to be Client Abandonment, which is a severe violation of policy and subject to immediate termination.
  • Timesheets. The client or a designated responsible person in the family must sign the Timesheets on a daily basis. Timesheetsmust accurately reflect the hours worked and must be mailed or brought in to the PRC office by the Tuesday morning after the week in which work was completed. Timesheets must accurately reflect the care rendered to the client arid match the Plan of Care authorized by the Nurse. Paychecks are given out every Friday. Falsifying timesheets is a violation of the AGENCY policy and will result in immediate termination.
  • Assignments. Only the employee assigned to the case is allowed to be present in the client’s home. The employee cannot assignhis/her case to another person. All assignment changes must be done through the PRC office. The employee is not allowed visitors during work hours, as this is a violation of the client’s right to confidentiality; The employee will not provide services for the client other than those specified on the Plan of Care. The employee will not work privately for the client.
  • Client Finances. The employee will not borrow money from the client, will not accept cash from the client or family as a bonus ortip for the work done, and will not perform extra services from payment.
  • Client Confidentiality. The employee will not discuss the client or his or her family with anyone other than an authorized personfrom the AGENCY or the vendor agency.
  • Employee Confidentiality. AGENCY doesn’t release our employees’ personal information (including telephone numbers -or work location) to clients or any other people other than by Court order or to Governmental agencies as provided for under the law.
  • Personal Issues. I will not discuss personal issues that I may have with the client of his or her family. Any personal issues that Ihave that need to be addressed will be done so with the designated person from the agency.
  • Employee Impairment. The employee will not report to work intoxicated, or under the influence or in possession of illegal drugs.AGENCY reserves the right to screen any employee for drug or alcohol usage on the job. If the employee is told to report for drug or alcohol testing the employee must do so at the time designated by AGENCY staff and refusing to do so will result in the immediate termination of employment and the automatic assumption of impairment.
  • Personal Contact. The employee will not provide the client with his/her personal telephone number or address. All contact mustbe through the AGENCY office only.
  • Employee Illness and Emergencies. Employees who are ill prior to their assigned shift must contact the agency as soon as theyknow that they are unable to report to work. If an employee becomes ill during work or has a personal emergency the AGENCY office must be notified immediately. Employees are not to leave a client unless they receive prior authorization from the AGENCY office, and to do otherwise is client abandonment.
  • Change in Client Status. If your client is hospitalized, transferred, changes residencies, or goes on vacation you must notify AGENCY immediately, AGENCY does not pay for services performed outside the client’s residence.
  • Requesting Time-OFF. If you wish to request holiday or vacation time, you must – do so in writing at least 2weeks before the date you want your leave to start. Time off is granted based on availability and client coverage.
  • Medication Administration– No prescribed medication is to ever be administered to any client and no invasive procedure (enema, douche, etc.) is allowed.
  • Lost Checks The Company is not responsible for lost checks. The employee will incur a $30.00 stop payment for lost checks.
  • Emergencies. Call 911 immediately in the event of any medical or safety emergency.
  • Pay Rate. The New York State minimum wage increased on December 31, 2019. In New York City, it is now $15.00 per hour for all size businesses. In Nassau, Suffolk and Westchester counties, it is $13.00 per hour. In the remainder of the state, it is $11.80 per hour.  The weekend and Holiday rate for NYC boroughs sustains 15.85/hour.
  • Paid Time Off. All full-time employees shall receive a total of eight (8) paid days off, with each day being calculated as such employee’s regular rate of pay multiplied by eight (8) hours, beginning on January 1st of each calendar year. Employees that work 20 hours per week or less shall receive a total of four (4) paid days off multiplied by eight (8) hours, beginning on January 1st of each calendar year.
